**NEW PATIENT INFORMATION**

The Executive Director will contact all new patients. We do not treat patients that we have not seen. We will not call in prescriptions or offer medical advice for patients prior to their initial visit.  

 

Please do not come in to roster as a new patient and expect to be seen that day.

We need to obtain your medical information to facilitate your transfer of care to our clinic.

 

Please bring your medications in at every visit.

If you have been to the Emergency Department or Specialist, and have been given a new prescription we may not be aware and we will need to update your profile. You must be seen in order to have prescriptions renewed. pharmacists are now able to give you a month supply of some of your prescriptions if you are completely out of medication. Please plan ahead for renewals to ensure your prescriptions are up to date.

Missed Appointments

If you are unable to attend your scheduled appointment, please call and either leave a message or talk to the front office staff. Not showing for an appointment is a wasted appointment; we cannot keep providing appointments to patients that do not show for their appointment.

Our nurse does lab work Tuesdays from 8:30 a.m. to 3:15 p.m.

This Health Passport encourages you to record your continuing health. It is intended to be used as a guide for ongoing health, wellness and follow-up of any Chronic Disease Process.

This passport gives you information about early signs of possible concerns regarding your health that you need to discuss with your Family Health Team Providers.

HOW TO USE YOUR PASSPORT

The Health Passport is designed to help you record important information. Keep it on hand, take it with you when you visit your Doctor or other Health Care Professional. Please make sure all entries are made accurately.

Your Family Health Team is available to assist you when entering your initial data.

 

We try to provide all patients the time that they require. For this reason, we kindly request your patience and understanding should a delay or rescheduling become necessary.

 

Our Team provides house calls and Palliative Care, so sometimes the providers are called away to look after these patients. Our geographical area is quite large and all of our patients are important to us, so there may be times when absence is necessary.
